Available only for Blue Cross Blue Shield of Alabama members with State Government, Local Government, or PEEHIP plans. Your Blue Cross Blue Shield Contract Number will begin with EIB, LGB, or EDU. Wellness screenings are a requirement for these insurances.

We are open Monday through Friday, 8:30 AM – 6:00 PM. We are closed on Saturday and Sunday.
